Welcome to Tucson Womens Center’s Influenza Vaccination program. Deciding to vaccinate your self and/or your family can be a very confusing process. Not everyone wants or needs the flu shot. We encourage you to consider all options before deciding if influenza vaccination is what is best for you. If you elect to have the vaccination for your self or your children, you will need to read the Vaccination Information Sheets (VIS’s) for the corresponding vaccine. Federal law requires that you read this information prior to your vaccination. Reading this information will also help you decide if you are in a group at higher risk for flu and whether you should receive one or both vaccinations.

To expedite your appointment, print, read, and sign the screening and consent form that corresponds to the vaccination you are requesting. If you would rather not print these forms, you can fill them out at our office.
